About the event

A person-centered support plan (PCP) is an individualized written or electronic resource that focuses on an individual’s preferences, personality, routines, and support needs rather than primarily on his or her disability.

The PCP develops services and supports based on the person’s priorities and strengths rather than problem areas.

Course Feedback

If someone else asked you, "Should I take this course?" What would you tell them?

· 1000% YES!!

· YES! Be ready to learn a lot, but also know you can take whatever information resonates with you wherever you are in the planning phase - the most important thing is to learn to think in a person-centered way

· YES! I’ve already been spreading the word!

· Yes! You will be surprised at how much you haven't included in your plan.

· Very important to learn about how to think about person centered planning

· Yes, of course. I would say it gave me a lot of frameworks to write and understand a PCP. It is an evolving process. · It was overwhelming to think about how to create a plan and would drain my energy. I have used the frameworks, and (now) having something as a plan to begin with gave me a big relief.

· Absolutely, it is as important as life insurance

· Take it and it will benefit your life and the lives of your loved ones

· YES, of course! And while this can be overwhelming the earlier the better.

· Definitely. It helped me to a lot, by giving framework to put my thoughts down, as well as bringing a shift in looking at things, and what is needed and important to the person. I think this thinking helps to build more positive connections with people who are involved in helping the person.

· DEFINITELY!!!!

· A 100% Yes! If you want to learn to advocate for your loved one in your presence and absence.

What is one "gem" you took away from this course?

· No detail is too minor.

· Use of multiple, focused one page profiles

· Important to and. Important for.

· I can actually do a complete plan even if it will need updates later,

· We so need to be specific, others input, Trudy’s help with tech help

· The way I have to explain, so that it can be understood by a person who does not know him. · As well as write the specifics in detail but in easy readable points.

· Important To/ Important For

· What a Person-Centered plan means and the paths to achieve it?

· Just get started and put this together

· Communicate in a way that gives importance to the person

· Trudy is a gem

· Language/wording!

· Outcomes are different from goals.

· One page description and the opportunity to have met Trudy Grable

"For me, the regularly updated person centered plan is critical for others in knowing what Lauren wants and doesn't want in her life; what I and others who care about Lauren have learned is important to and for her and how she best benefits from support.

The plan enables others to support the life my daughter wishes to have. It gives me peace that when I die, her life will continue to be rich" ~Trudy.

P.S. Spelling does not count in this course!
